NEW PATIENTS
If this is your child’s first visit to Thomas Pediatrics, please bring the following documents with you.
1. Insurance Card
2. Complete vaccination record
3. Medical records from previous doctor or hospital (if available)
4. Any other relevant medical information
We accept most insurances. If your insurance plan requires you to choose a Primary Care Physician (PCP) for your child, please make sure you have Dr. Thomas listed on the child’s insurance card.
